Tapestry 360 Health (formerly Heartland International Health Center ), works to create a “medical home” that will meet the diverse needs of those who are struggling to find affordable health care, particularly the uninsured, underinsured, immigrants, and refugees. Our vision is to be a national leader in community-based health care by advancing innovative service models and patient-centered best practices.
