THIS TRUST FUND IS A COLLECTIVELY BARGAINED TAFT-HARTLEY MULTIEMPLOYER HEALTH AND WELFARE PLAN. IT PROVIDES HEALTH AND WELFARE BENEFITS FOR PARTICIPANTS AND BENEFICIARIES WHO ARE ELIGIBLE UNDER THE TERMS OF THE TRUST AGREEMENT.
